This class is an evidence-based program that incorporates the proven benefits of aerobic exercise and strength training to maintain good health and manage chronic medical conditions like diabetes.

Patients and clients with – or at risk of developing – diabetes and other chronic, lifestyle-related conditions will require medical clearance from their General Practitioner (GP) before participating in the program.
People with diabetes need to work with a team of health care professionals, including their doctor, diabetes expert and dietitian. They should also work closely with their doctors to check the eyes, kidneys, feet, blood pressure, blood lipids, body weight and blood glucose levels.
Maintaining a healthy lifestyle can also help reduce the risk of developing complications associated with diabetes. A healthy lifestyle means being physically active most days, maintaining a healthy weight, adopting healthy eating habits with fruit, vegetables and low-fat foods and not smoking.
People with Type 1 diabetes need insulin for the rest of their lives. Some people with Type 2 diabetes can manage their condition with regular physical activity, healthy eating and maintaining a healthy weight. Others may need tablets and/or insulin injections too.

Eligible clients must have Type 2 diabetes. Before referring clients, GPs must:
GPs are also encouraged to attach a copy of the relevant part of the client’s care plan to the referral form sent to the allied healthcare provider.
Clients referred for allied health group services under items 81100 to 81125 do not need to have a Team Care Arrangements (TCA) service (item 723). If the GP also wishes to refer the client for individual allied health services under items 10950 to 10970, this service must be in place to meet the eligibility requirements of those item codes.
People with Type 2 diabetes can receive Medicare rebates for group services provided by eligible diabetes educators, exercise physiologists and dietitians on referral from a GP.
Group services are in addition to the five individual allied health services available to eligible clients with chronic or terminal conditions.

When you see a health professional, it is important to recognise three main health levels: ACUTE, SUB-ACUTE and CHRONIC conditions and or/illnesses.
ACUTE is pain or injury that begins suddenly, typically within 1-2 weeks of trauma or injury. You may not even know what caused the acute condition; however, the pain can affect your functional ability.
SUB-ACUTE refers to when pain or injury has been relatively managed and the focus shifts to repairing tissue and mobilising the affected body part. This approach can be identified within 3-4 weeks in and can last up to 6 weeks post-trauma or injury.
CHRONIC refers to any condition or illness lasting 6 months or more. These ongoing issues affect your ability to function and perform your daily activities normally.
Exercise physiologists treat mostly SUB-ACUTE and CHRONIC health conditions, injuries or illnesses. After a comprehensive assessment, clients have a prescribed exercise program, akin to a doctor’s approach and prescription.
Although many clients are prescribed medication to manage a health condition, with TCR, there is a strong focus on behavioural changes. These involve self-management strategies through education tactics to help you return to a healthy functioning state.
Physiotherapists primarily treat ACUTE and SUB-ACUTE patients using physical techniques such as manipulation, massage and dry needling. They also focus on targeted exercise to increase mobility and range of motion through weak joints or injured body parts.
Often, a physiotherapist and exercise physiologist may see a client for the same condition but at different stages of the rehabilitation program. For example, someone who hurt their back while lifting something may initially see a physiotherapist to undergo manual therapy, manage symptoms and control inflammation, then move on to an exercise physiologist to restore movement and increase strength.
