What is a chronic medical condition?
A chronic medical condition has been (or is likely to be) present for six months or longer, for example:
- asthma
- cancer
- cardiovascular disease
- diabetes
- musculoskeletal conditions
- stroke.
Understanding Chronic Disease Management (CDM)
The Chronic Disease Management (CDM) program is for clients who require a structured approach. Our CDM rehabilitation services consider the client’s medical condition and care needs per the general guidelines set out by the Medicare Benefits Scheme.
Clients with chronic medical conditions and complex care needs being managed by their GP under a GP Management Plan (item 721) and Team Care Arrangements (item 723) are eligible for Medicare rebates for certain allied health services on referral from their GP.
